Work as a doctor in Germany

More than 68,000 doctors trained abroad already work in Germany. The route is long mainly because of the language: you need a full medical licence (Approbation), and for that you must speak medical German at C1 level and, with a degree from outside the EU, pass an oral exam on clinical knowledge.

New from 1 November 2026: a federal law (BGBl. I Nr. 225, 28 July 2026) makes the Kenntnisprüfung the standard route for doctors with a degree from outside the EU. The slow document comparison (Gleichwertigkeitsprüfung) then only happens if you apply for it. Applications filed before that date usually stay under the old rules.

Where did you qualify?

Degrees from the UK (since Brexit), the USA, Canada, Australia and similar count as non-EU degrees: you take the same route as everyone outside the EU, including the Kenntnisprüfung.

Step by step

Each fee links to the page we took it from.

  1. 1

    Learn German to B2

    varies

Most states want a general B2 certificate (for example Goethe or telc) before they accept the medical language exam. Course and test costs depend on the provider.

  2. 2

    Apply for the Approbation

    set by each state

Apply to the licensing authority of the state where you want to work. Typical documents: diploma, transcript, passport, CV, certificate of good standing, criminal record and medical certificate. The authority tells you which translations and certifications it needs.

  3. 3

Pass the Fachsprachprüfung (C1 medical German)

    €595 in Lower Saxony

Run by the state medical chamber. In Lower Saxony it has three parts of 20 minutes each: a conversation with a simulated patient, a written doctor's report, and a talk with a colleague about the case. Source

  4. 4

    Pass the Kenntnisprüfung

    €980 in Lower Saxony

Oral-practical exam on the knowledge of a doctor finishing German medical school, focused on internal medicine and surgery. In Lower Saxony each candidate is examined for 60–90 minutes in a group of up to four. Expect several months of waiting for a date. Source

  5. 5

Optional: work on a temporary permit

    set by each state

While you prepare, many states can grant a temporary licence (Berufserlaubnis) to work under supervision, for up to two years.

  6. 6

    Receive the Approbation

    set by each state

With the licence you can work anywhere in Germany, for life. The authority charges its fee at the end of the procedure.

Not included: courses, translations, travel, visas and living costs.

Questions

Can I take the exams in English?

No. Both the Fachsprachprüfung and the Kenntnisprüfung are held in German.

Which state should I choose?

The state where you want to work. Fees, waiting times and the exam format differ between states, but the licence is valid in all of Germany.

Do I need a job offer first?

No. You can apply for the licence without a job, but a visa for job seekers or for recognition measures has its own rules – check with the German embassy in your country.
